Key Areas of Accountability / Essential Functions


  1. Communicate the department and agency mission, vision and strategic goals to all department personnel, creating an understanding and desire to contribute towards the achievement of those goals.
  2. Contribute to the leadership of the organization through participation in the decision-making process.
  3. Contribute to the long-term success of the department and agency through a commitment to staff professional development.
  4. Review and evaluate the effectiveness of existing programs, ensuring the fulfillment of obligations dictated by funding sources and/or specific contracts/grants.
  5. Responsible for meeting CARF, FSSA, BQIS, Medicaid, and Board of Health standards and other regulations and procedures.

Staff Supervision

  1. Coordinate with the AD and Human Resources the posting and recruitment of open positions.
  2. Responsible for training, coaching and performance management for assigned personnel, which includes the timely completion of performance reviews.
  3. Responsible for scheduling staff in a manner that provides needed staff coverage and minimizes overtime pay, while ensuring that IPP's and Person Centered Plan can be adequately achieved and documented.
  4. Responsible for effectively managing workflow and work assignments for assigned personnel as described in job descriptions.
  5. Responsible for the development of mutually cooperative relationships between departments that ensures the sharing of information.
  6. Responsible for resolving personnel conflicts and other related issues among or with assigned personnel.
  7. Responsible for adhering to and ensuring the adherence to all organizational policies and procedures.
  8. Responsible for keeping Human Resources apprised of any potential employee relations issues or performance issues that may result in corrective action.

Consumer Services/Case Management

  1. Obtain and/or complete all admission, enrollment, diagnostic and benefit paperwork for individuals new to our services.
  2. Facilitate annual ISP / IDT meetings and all other meetings as requested. Document individual strengths, interests, and needs to determine appropriate community involvement, and facilitate (if certified) or participate in the Person Centered Planning Process.
  3. Develop, oversee, and ensure the implementation of the IPP, ISP, Person Centered Plans, Behavior Plans and High Risk Health Plans, including the development of strategies that support these plans.
  4. Responsible for ensuring all staff are trained on the content of the consumer IPP, ISP's, Behavior Plans and High Risk Health Plans, and ensuring all staff understand their responsibilities in the implementation of these plans and the associated strategies.
  5. Coordinate monthly and quarterly reviews, as well as annual meetings. Submit appropriate documentation to team members, VR Counselors and others as determined by regulation or procedures and maintain thorough consumer records.
  6. Maintain consumer records per regulations and company procedures, conducting regular file audits and ensuring their completeness.
  7. Submit and review all monthly and quarterly summaries.
  8. Ensure that Associate Director is regularly informed of program progress, changes and critical issues.
  9. Present a professional image as a role model for consumers, staff and the community.
  10. Ensure staff is available and scheduled to provide all approved services.
  11. Obtain, review and approve records of time worked for all direct reports, ensuring that there is appropriate supporting documentation.
  12. Attend appropriate staff meetings, in-service training sessions, quarterlies and annuals when necessary.
  13. Ensure all medications are ordered in a timely manner, administered properly and documented accurately.
  14. Ensure that regular activities are scheduled that are aligned with the consumer's ISP and which contribute to a meaningful day.
  15. Provide personal care and transportation as needed. Must continue to meet drivers' insurability and physical requirements as outlined in Minimum Job Qualifications.

 Consumer Advocacy

  1. Maintain positive, on-going communication with consumers and their team members, which may include family members, support staff and other service providers.
  2. Maintain a positive and facilitative role with consumers to encourage greater independence and self-reliance.
  3. Assist consumers in exercising choices and developing self-advocacy skills.

Health and Safety

  1. Provide on-call support as needed. Respond to emergency situations or provide crisis intervention within 30 minutes of receipt of notification.
  2. Ensure the maintenance of a clean, safe and healthy environment for consumers.
  3. Conduct regular inspections and report maintenance issues promptly to ensure a clean, safe, and healthy environment for consumers, providing Director of intake, Marketing and Properties with a maintenance request and/or phone call if it is a critical issue.
  4. Ensure all safety measures, including fire and tornado drills, are implemented and appropriately documented, per regulations and company procedures.
  5. Monitor use and maintenance of agency vehicles, providing Director of intake, Marketing and Properties with notification of maintenance issues.
  6. Ensure that balanced and nutritious menus are planned based on recommendations and guidelines established by the contracted Dietitian - with the input of consumers, and a three day emergency food supply is maintained.
  7. Coordinate with the appropriate health care professionals to ensure consumers' medical needs are met according to Board of Health Guidelines.
  8. Ensure all paperwork regarding reportable incidents is submitted on a timely basis.

Financial Management

  1. Oversee purchasing, storage and handling of groceries, household items and consumer's personal belongings.
  2. Ensure the home operates within regulations and budgeted guidelines.
  3. Oversee appropriate use of both consumer and Tangram monies.
  4. Ensure the maintenance of consumer financial and petty cash records in accordance with agency guidelines.
  5. Ensure timely submission to the Finance Department all documentation required to complete billing of services to appropriate funding sources.
  6. Submit reports and maintain resident records per regulations and company procedures.
  7. Oversee and implement entitlement systems as it relates to Social Security, Medicaid, and Medicare.
  8. Ensure that the program financial processes operate within agency policy, budgetary guidelines and external regulatory guidelines.



Minimum Job Qualifications

  1. Education: Bachelor's degree in rehabilitation, psychology, or other related major.
  2. Work Experience: 2 years experience working with persons with developmental disabilities.
  3. General Requirements: Must have a valid driver's license, acceptable driving record, meet agency's driver insurability requirements, reliable vehicle, and up-to-date automobile insurance.
  4. Working Conditions: This position requires significant bending, lifting and stooping. Must be able to lift/transfer individuals.
  5. Training Requirements During Employment: Must maintain CPR and first aid certification, must stay current on all other training as defined by state, accreditation body or organization guidelines.
